I am currrently accepting new clients. Appointments are via telehealth; in-person appointments are available on an as-needed basis. All clients must be in Oregon for appointments. 

NEW CLIENTS

Please fill out the Prospective Patient Form to begin the intake process. We will call you if we have further questions. Otherwise, you will receive an invitation to the Portal. Once the Portal account is created, we will send you intake paperwork and questionnaires through the Portal. Once the paperwork is complete and insurance is verified, we will schedule an appointment. 

YOUR FIRST APPOINTMENT

The first meeting typically lasts 60-90 minutes. Please complete all forms and questionnaires before the appointment. We will discuss your medical, psychological, and social history; current symptoms and stressors; risk and protective factors; and treatment goals. We will decide whether we are a good fit. We may begin a treatment plan, or further assessment and discussion may be necessary. An intake appointment does not guarantee ongoing care or medication prescriptions. You become an active client when fill a prescribed medication or attend a follow-up appointment.

TELEMEDICINE TECHNICAL REQUIREMENTS

• A secure, private computer with a functioning camera and microphone. Public computers, such as those in libraries, are not acceptable. 
• Tablets and mobile phones may work but the telecommunication software works best on a desktop. 
• A private high-speed internet connection. Data service through a mobile phone may not be adequate. Public WIFI hotspots are not acceptable.
• Chrome or Firefox are the preferred web browsers.
• Meeting from your car may afford some privacy but may cause connectivity issues.
• A quiet, private location where you will not be uninterrupted. 
• Please download apps and test devices at least 48 hours before your appointment.
